Simple steps for claims process

Download and fill in the claim form


Here are the list of supporting documents for your claim:
1. Claim Statement
2. Medical Report
3. Original Bill & Receipt
4. Investigation Report

Submit the completed claim form and the listed supporting documents to AXA or your Agent

Upon receipt of full documents, AXA will revert with claim decision within 7 working days

e-claim payment within 7 working days from your claim approval date

Note:
The above steps are applicable for:-
a) Admission to non-panel hospitals.
b) Outpatient treatment due to minor injury sustained in an accident.
c) Outpatient treatment prior to admission and/or follow-up treatment after discharge from the hospital.
d) Day surgery whereby you are not required to stay overnight at the hospital.
e) Outpatient cancer treatment and outpatient kidney dialysis.
f) Hospitalisation outside Malaysia.

Send the admission letter via email to your assigned Third party Administrator (TPA) at least 1 day before hospital admission.

Request for a copy of your guarantee letter from your assigned TPA and bring it along for your appointment.

For Cashless Admission to Panel Hospitals – applicable only if you have our AXA HealthCare Card You may call the hotline number 1300-88-9979 printed on your medical card prior to your hospital admission for any inquiry.

Present your physical or e-medical card at the hospital.  The hospital will assist to contact your assigned Third party Administrator (TPA) for the hospital admission.

Submit completed claim form and original supporting documents within 30 days after medical visit via:
1. IHP Mobile App
2. claims@axa.com.my
3. Your agent

Upon receipt of complete documentation and information, AXA will revert with claim decision within 14 working days.

Payment will be made via Bank Transfer within 5 working days from your claim approval date.
